The Challenge

Our Intervention

The intervention was a multi-level strategy focused on embedding accountability, communication, and visual management into daily operations:

  • Executive Support: Executives were coached on Rounding, Active Listening, and Inquiry techniques to gain timely, accurate information, enabling better decision-making and support for frontline teams.

  • Mid-Level Accountability: Mid-level leaders were trained on consistent Rounding, Daily Operational Huddles, and Performance Coaching to drive accountability at the unit level.

  • Frontline Empowerment: Frontline leadership and teams were taught to use visual management tools like Run charts, Pareto charts, and leading indicators to track and own their performance.

  • Cultural Foundation: Established that Psychological Safety was paramount; errors were treated as opportunities to learn, not to punish, which was critical for increasing transparent error reporting.
